When I first started working as a physiotherapist I was shocked to realise how many of my patients with back pain had been told completely different things by different care providers. I also realised that even my colleagues didn’t always agree on how to best manage back pain. Some felt that hands on treatments were best, others preferred a more active approach, and some believed it was how you felt about it that made the difference. Even I was struggling to make sense of it, so how on earth was someone with no training going to work the problem out?
I was seeing hundreds of patients with back pain and I wasn’t 100% sure how I should be treating them. I started to try and draw all the different strands together to see if there was a clear answer to the question of how to manage back pain. I read as much of the literature as I could and discussed the problem at length with my peers, but it was impossible. Twenty years ago there was very little guidance or evidence that pointed people in the right direction. It was very hard to change the habits and ideas of a lifetime too…some clinicians just wanted to carry on doing what they had always done and many patients just wanted someone to fix it for them. So I struggled on trying to find the best way to manage back pain for years...
…until the publication of some important guidelines in 1994 by the Clinical Standards Advisory Group (CSAG) finally provided some research based guidance saying that it was best to try and remain active if you had back pain. Despite this, it was still difficult to find a way to help people that didn’t involve going down either one treatment road or another, there didn’t seem to be a way that linked everything together. I kept searching for answers. I began to study the research about pain and the way in which our feelings and surroundings can influence back pain. I was amazed by Gordon Waddell's work 'The Back Pain Revolution' which looked at the relationship between pain and distress. I also studied David Butler and Louis Gifford's work on the nervous system and attended training courses with them both. This was the final piece of the puzzle for me which helped me join the dots and understand why a fragmented approach to back pain was often unhelpful.
My next problem was how to explain all these complicated issues to my patients? I started to develop my own method and found a successful way of explaining the bigger picture and why they needed to take lots of different factors into account when treating their back pain. I broke their treatment down into three easy to follow steps:
1. Reducing some of the stresses and strains that their back was subjected to
2. Making their back fitter so that it could handle whatever they threw at it
3. Making sure their thoughts and feelings were not hindering their recovery
After that I started to see much better results. My patients were clearer about what they needed to do, and I felt happy that I was using the best research to deliver a really comprehensive treatment programme that didn’t force either me or my patients down a one way street.
Since this breakthrough I’ve refined and developed my ideas to make sure that I haven’t missed any new research. Over the years the evidence for keeping active has strengthened, and so I have emphasised this.

The latest piece of the puzzle slotted into place a few years ago. I was struggling to think of a way to encourage people who had changes on their scan and X-ray to not feel despondent. I needed to explain why this is just one piece of the problem and that by taking care of all the other pieces these changes can be overcome. I racked my brains for a way to explain the fluidity of the relationship between all these pieces, when I hit upon the idea of water filled pools to represent the six main factors involved in back pain. I checked it out with my colleagues to see if they thought I had lost my senses, but they all thought it was a brilliant way of describing a very complex issue. I’ve even heard them describing the six different ‘pools’ to their patients!
It was at this point I decided to write a book. I get very upset by the amount of poor quality 'information' available for back pain sufferers and I wanted to help people I would never get the chance to see. Trying to distil all my years of experience, all my training, my reading, the thousands of discussions and problem solving sessions with my colleagues into one text was really hard.
